For Patient requests, please go to http://medicopy.net/patients and fill out the records request form.
PLEASE DO NOT SUBMIT PAYMENT TO MCCORMICK AMBULANCE.
ALL PAYMENTS MUST BE SUMBITTED TO MEDICOPY.
CONTACT MEDICOPY FOR BALANCE OF PAYMENT FOR YOUR REQUEST.
For document requests that did not include an AMBULANCE transport:
Use this form to request a copy of your patient care report (PCR). If you provided your email address to us when the PCR was being filled out, we can send a copy to that address. If you did not provide an email address, you must include a copy of your government identification (Drivers license or other government I.D.)
